They provide not only custom textures, which look good and give you bragging rights for having a cool looking sword, but also have custom properties which modify the items power significantly, like extra damage or attack speed.

Creating these assets however is a tedious process. The creators need to (or hire someone) to draw these assets, then configure a resource pack (minecraft way of delivering these custom assets), and finally add it to their server. This is a tedious process and to even add one more asset to their server, they need to all this over again.

If item properties are also considered, there is an extra layer of balancing the item as an item too powerful will break the server balance and ruin the game. An under-powered item won’t be used at all
